Type 2 Diabetes is commonest form of diabetes in India. It is seen commonly in individuals who are above 40 years. Some of the risk factors for developing this type of diabetes are obesity, sedentary life style, unhealthy diet and lack of exercise. In this form of diabetes action of insulin is defective due to insulin resistance.This leads to increase in blood sugar and development of diabetes symptoms.
In Type-1 diabetes, the body completely stops producing any insulin. Insulin is usually produced by cells in the pancreas and these cells are dysfunction in type 1 diabetes due to an attack by the person’s own immune system. Therefore patients with type 1 diabetes must take daily insulin injections to survive. This form of diabetes usually develops in children or young adults, but can occur at any age. It is relatively less common as compared to type 2 diabetes.
Diabetes diagnosed for the first time during pregnancy (gestation) is Gestational diabetes. Gestational diabetes mellitus (GDM) is defined as any unusually high glucose level diagnosed with onset or first recognition during pregnancy. Depending on individual case, it is decided whether insulin or only diet modification is used for treatment.
Diabetes is the major cause of kidney disease. As diabetes is a progressive disease, the prevalence of diabetes related kidney disease increases with the duration of diabetes. Kidney disease in diabetes can range from mild derangement of kidney function to major kidney failure leading to dialysis.
To effectively manage kidney disease in diabetes patient, a team approach of diabetologist and nephrologists (kidney specialist) is required.
Patients with diabetes commonly have musculoskeletal problems (diabetes hand, frozen shoulder, carpal tunnel syndrome, and others). We screen, identify and manage these conditions which produce considerable morbidity. These are seen in both type 2 and type 1 diabetes. In addition, since type 1 diabetes is an autoimmune disorder, there is a chance of overlap with other autoimmune diseases like arthritis, celiac disease, lupus. Rheumatology can also deal with such other systemic autoimmune problems. Patients with diabetes are also prone to osteoporosis and osteoarthritis, common degenerative conditions.
To effectively manage this in a diabetes patient, a team approach of diabetologist and rheumatologist is required.
Stress aggravates diabetes. Peripheral Doppler to measure Ankle Brachial Index (ABI) : ABI is an important measure to diagnose peripheral vascular disease. The index is important and compares the blood pressures of the arms and legs to give a ratio that can suggest various severity of peripheral vascular disease.
Biothesiometry : It is a simple and economical tool for measurement of Vibration Perception Threshold. It is predominantly used for foot care to see understand sensitivity of the foot (sensitivity threshold) and to vibration diagnosing peripheral neuropathy.
DEXA (Dual-energy X-ray Absorptiometry) : DEXA is used to measure bone mineral density or Bone mineral content for screening & diagnosis of osteoporosis. This helps in identifying those who are at high risk for fracture which can be treated. It is particular useful in chronic diseases like chronic kidney disease, diabetes etc. Additionally, it also measures body composition (fat %, lean % of the body) which has changed the perspective of measuring obesity.
